Covid 19  Studio regulations  

We ask all our clients to follow our Covid 19 studio polices with the following:
  • Please call the studio 10 minutes before your arrival this is so we can prepare for your arrival.
  • Washing/ disinfecting your hands when entering the studio.
  • Your temperature will be tested on your arrival.
  • You will required to fill in our Covid 19 informed consent form and Health Questionnaire. 
  • Wearing a mask at all times in the studio.
  • Remaining socially distancing when ever possible, we will ask you to sit in a designated seat until your tattoo artist is ready for you. 
 
Epona Tattoo Health Questionaire.

If you have been exposed to a communicable disease, you may spread the disease to the artists or other clients. Therefore, Prior to each appointment we will be asking the following questions to reduce the chances of transmission. 

Have you or other recent acquaintances tested positive for or been diagnosed as having COVID-19 or any other communicable diseases
YES or No 

If yes, when? Date:

Do you, others accompanying you to today's appointment or other recent acquaintances have:

A fever (defined as above 37.8 degrees Celsius) YES   or NO
A cough  YES or NO
Shortness of breath and or trouble breathing YES or NO
Persistent pain, pressure or tightness in the chest YES or NO

I understand that if the answer to any of these questions is YES. I will asked to reschedule today's tattoo appointment.

Thank you for your continued trust in our studio. 

As with the transmission of any communicable disease like a cold or flu, you may be exposed to COVID-19, also known as the Coronavirus at anytime or in any place. 

Be assured that we have always followed government regulations and recommended PPE and disinfection protocols to limit transmission of all diseases in our studio and continue to do so. 

Despite our careful attention to sterilization, disinfection, and use of personal barriers, there is still a chance that you could be exposed to an illness in our studio. Just as you might be at  a Gym, Supermarket or favourite Restaurant. 

Social distancing nationwide has helped reduced the transmission of the Coronavirus. Although we have taken measures to provide social distancing in our practice, due to the nature of the procedures we provide, it is not possible to maintain social distancing between you and your artist and even sometimes other artists, staff, or clients at all times. 

You the client also agree to follow our COVID-19 studio polices with the following:
  • Wearing a mask at all times the studio.
  • Washing/disinfecting your hands
  • Remaining socially distancing when ever possible, to help us better serve you and reduce the risk of exposure to yourself and others that may be present. ​ 


          
Epona Tattoo Sanctuary Studio is constantly working within the current government guidelines. To help reduce any risk or exposure.
